Ageing in Place Pathfinder GM project    

Healthy Me Healthy Communities’ partnership with Southway Housing Trust, to deliver services in line with the The Greater Manchester Ageing in Place Pathfinder, demonstrates our commitment to tackle inequalities in ageing. Working alongside partners across GM neighbourhoods, the Pathfinder is delivering community-led responses of everyday support for residents aged 50 plus.

In celebration of two years of partnership working, the ‘Ageing in Place Pathfinder GM Report’ shared case studies and the importance of space and place for creating age-friendly communities.

And now, after 3 years, as the project comes to a close, we share our impact report on the difference this work has made to the lives of older people in Gorton and Abbey Hey.

Gorton Central: An Age-friendly Space   


Healthy Me Healthy Communities was commissioned by Southway Housing Trust to lead on age-friendly activities for the residents of Abbey Hey and Gorton. Extensive community consultation concluded a lack of age-friendly spaces and social activities for older residents of Abbey Hey. The location of Gorton Central, adjacent to Southway Housing Trust’s new older people's supported accommodation complex, meant that the community centre was strategically positioned to deliver a variety of neighbourhood projects.

Celebrating 3 years of our Age-friendly Space   

We’re delighted to share the achievements of our Ageing in Place Project (AIPP), which is now coming to a close after three years of dedicated work. The project set out to explore how older people can live independently, safely, and happily in their own homes for longer and together we’ve achieved so much. Through strong partnerships with local residents, health professionals, and community organisations, we:


  • Supported older people to shape services that truly meet their needs


  • Delivered workshops and activities that promoted wellbeing, digital skills, and social connection


  • Created safe and welcoming community spaces where people could access advice and support


  • Shared valuable insights with researchers and policymakers to influence future approaches to ageing well


The legacy of AIPP will continue through the relationships built, the learning gathered, and the lasting impact on participants’ lives. We’ve captured the full story of the project, including case studies and recommendations for the future, in our final report.
